The PVH Health Information Management Department is dedicated to caring for your medical record and protecting its confidentiality. The staff is responsible for ensuring that the record is accurate and complete. Although we have transitioned from paper to electronic records, we can provide you with a copy of your record whenever you request it either on paper or in disc format. We require that you sign an authorization to release information. You may obtain an authorization by downloading the document below, calling the H.I.M. Department, emailing or coming into the office. Once the completed authorization is received by H.I.M. (Medical Records) Department, we will complete your request as soon as possible. Please be sure to add contact information so that we may get in touch with you once your request is complete.
