Case Management & Social Work

The Case Management Department acts as a resource to the patients, the medical providers, the hospital, and the community. Its role is to facilitate effective follow-up and wellness after discharge from the facility. The Case Management team works with all hospital departments in the process of assessment, planning, accessing, coordinating, and implementing care for the patient.

The PVH Social Services program helps patients and families cope with the physical and emotional trauma of illness and/or injury. It helps them figure out their options and assists them in making difficult decisions. Social Services works with all hospital departments in the process of assessment and planning. It often deals with patient and family rights, privacy, confidentiality and informed consent.

An open referral system exists allowing access to social services from any source. Hospitalization can be stressful, and each patient and family responds to these difficult times in a unique way. Social services offer the following:
