• Periodic Health Screenings

1. Complete the Occupational Health Scheduling Sheet, available above, to indicate what services need to be preformed.
2. Attach a copy of the employees job description
3. Fax the completed Occupational Health Sheet AND the job description to 207-403-7698
4. Our staff will reach out to the employee to schedule using the number you provided on the scheduling sheet.
